CA125是1981年由Bast等從上皮性卵巢癌抗原檢測出可被單克隆抗體OC125結(jié)合的一種糖蛋白，來源于胚胎發(fā)育期體腔上皮，在正常卵巢組織中不存在，因此zui常見于上皮性卵巢腫瘤（漿液性腫瘤）患者的血清中，其診斷的敏感性較高，但特異性較差。黏液性卵巢腫瘤中不存在。80%的卵巢上皮性腫瘤患者血清CA125升高，但近半數(shù)的早期病例并不升高，故不單獨用于卵巢上皮性癌的早期診斷。90%患者血清CA125與病程進展有關，故多用于病情檢測和療效評估。95%的健康成年婦女CA125的水平≤40U/ml，若升高至正常值的2倍以上應引起重視。另外CA125也可見于結(jié)核性腹膜炎患者的血清檢查中，且CA125水平呈數(shù)十倍升高，在卵巢癌術前應明確排除結(jié)核性腹膜炎可能。

診斷檢查
病原體檢查
取自患處的標本作直接涂片或培養(yǎng)，涂片可見菌絲或曲霉菌孢子，培養(yǎng)見曲霉菌生長。曲霉菌是實驗室常見的污染菌，必須反復涂片或培養(yǎng)，多次陽性且為同一菌種才有診斷價值。
病理組織檢查
取受損組織或淋巴結(jié)活檢，可根據(jù)真菌形態(tài)確診。尤其對播散性曲霉菌，可及時作出診斷。
治療方案
一般治療
1、積極治療原發(fā)并，去除病因。
2、嚴格掌握抗生素、糖皮質(zhì)激素和免疫抑制劑的用藥指征，盡可能少用或不用這些藥物。
3、加強護理和支持療法，補充維生素和微量元素。
抗真菌治療
1、制霉菌素
（1）局部用藥：可制成油劑、霜劑、粉劑、溶液等，濃度為含制霉菌素10萬U/g或/ml基質(zhì)，依患者具體情況援用一種劑量一型局部涂擦，每日2—4次。
（2）口服：腸道念珠菌病可給予制霉菌素口服，新生兒每日20—40萬U，2歲以下每日40—80萬U，2歲以上每日100—200萬U，分3—4次飯前服用，療程7—10日?？诜灰孜?，全部由糞便排出。不良反應有抗原抗體、嘔吐、輕瀉。
（3）霧化吸入：適用于呼吸系統(tǒng)念珠菌病，制霉菌素5萬U溶于2ml0.9%氯化鈉溶液中霧化吸入。
2、二性霉素B為多烯類抗生素
與真菌胞膜上的固醇類結(jié)合，改變膜的通透性，使菌體破壞，起殺菌作用。是目前治療隱球菌病、組織胞漿菌病和全身念珠菌病的*藥物，對曲霉素菌病效果較差。
（1）靜脈滴注：開始義用小量，每日0.1mg/kg，如無不良反應，漸增至每日1—1.5mg/kg，療程1—3個月。靜注時用5%葡萄糖液稀釋，濃度不超過0.05—0.1mg/ml，緩慢靜脈滴注，每劑不少于6小時滴完。濃度過高易引起靜脈炎，滴速過快可發(fā)生抽搐，心律時常、血壓驟降，甚至心跳停搏。
（2）椎管內(nèi)注射或腦室內(nèi)注射：限于治療隱球菌性膜的病情嚴重或靜脈滴注失敗的病例。兒童鞘內(nèi)注射，*0.1mg用蒸餾水（不用0.9%氯化鈉溶液）稀釋，濃度不超過0.25mg/ml（偏稀為宜）或?qū)⑺幬锱c腰穿時引流出的腦脊液3—5md混合后一并緩慢注入。以0.5mg為止不低超過0.7mg。療程一般約30次，如有副作用可減量或暫停用藥，腦脊液內(nèi)藥物過多可引起蛛網(wǎng)膜炎而腦脊液細胞增多，暫時性神經(jīng)根炎、感覺消失、尿潴留、甚至癱瘓、抽搐，如及早停吆五，大多能緩解。

General treatment
